Rescue Services
For adult children who live out of state, knowing that mom & dad are well can be troublesome. Living at a distance makes it impossible to know if their nutritional, safety and hygiene needs are being met. This is an area we can help the most. We consider this area one of the most important services we offer. We can “rescue” aging parents from any unsafe situation by providing the care and direction they need.
Through our comprehensive care management program, we will conduct a face to face meeting with your parents. This will allow us to ascertain the necessary information to lead them and you in the best direction. If they are in need of home assistance, meals, or home improvements we will recognize and coordinate all necessary services. We will also oversee the services to assure the best care is being delivered to your parent.
If mom or dad would benefit from downsizing, we will locate the best housing options for them, whether it be an apartment setting, or long term care setting such as assisted living or skilled nursing. We will be their advocate to be sure they are receiving the highest quality of care. We will also take care of the actual move, while coordinating the furniture and set up in the new location. We can also assist with setting up realtors for the sale of an existing home.
We act as a liaison between you & your parents to give you the piece of mind that they are ok. We will become your eyes and ears while assuring you that mom and dad are living as safely as possible in the best environment to meet their needs.
Care Management Services
As experienced professionals, Solutions for Independence provides assessments, care coordination and education, care monitoring and advocacy to older adults and their families.
We recognize that caring for a loved one can become stressful and often time-consuming. In this situation, a Care Manager becomes an “extended family member” who can offer a tremendous amount of empathy, compassion and direction.
Very often children take on the responsibility of looking after aging parents or loved ones. While this is admirable, the increasing complexity of health and financial issues facing today's elderly can become overwhelming. And the stress of trying to negotiate the maze of financial and health-related issues can often put a strain on the parent/child relationship… Our team can help.
Our specialized Care Managers are well versed in a variety of areas such as: health care, counseling, long-term care decisions, crisis intervention, and housing and long-term care placement. In turn, we advocate for those we service.
Our Care management services include:
- completing a comprehensive assessment to determine the appropriate level of healthcare and living needs
- developing a plan of care with family and friends while coordinating medical, supportive, social, legal and financial services
- attending physician appointments, medical tests, treatments, therapies and Emergency Room visits
- preparing applications for benefits: including, but not limited to Veterans Administration, private insurance and government programs
- locate reputable financial assistance, realtors, as well as legal help
- When children live at a distance or lack the considerable time it takes to care for a loved one, a Care Manager can even do shopping, manage finances, sort mail, transport to & from physician appointments/surgeries, assist with moving, and protect consumers from increasing scams.
Fiduciary services
Solutions for Independence provides fiduciary services by serving as Agent or Attorney-in-Fact through the Power of Attorney document for finances and/or healthcare. The Power of Attorney document grants the Agent the power to act on behalf of the senior.
The grant of power can be specific or extensive depending on the needs and preferences and most often include financial management, care management and healthcare decision making.
Long-term care Placement
If placement in an extended care facility is necessary, we will guide you through the entire process:
- identifying living options: retirement residence, assisted living, Alzheimer's and/or memory care, or skilled nursing care
- scheduling tours, completing admission paperwork and coordinating the move
- preparing a personal residence for sale or closing an apartment
- conducting monitoring visits to ensure safety, comfort and the appropriate quality and level of care
